Panther Pride Preschool Registration Forms

FREE AND REDUCED MEAL APPLICATION: After you have completed the online registration, your building office personnel will set you up with your own log-in information. After this is done, if you would like to apply for free or reduced meals, you may do an online application with this link: Application for Free/Reduced Meals. Please allow 48 hours for the Food Service Office to get back to you regarding your eligibility. Guidelines for free and reduced meals are explained in the Informational Letter for Seamless Summer Options, Free and Reduced Frequently Asked Questions.

The application is also available in a printable form: Application for Free/Reduced Meals (printable). and can be returned to the Food Service Office for eligibility to be determined. These forms are also available at any of the buildings or the Food Service Office.

INSTRUCTIONAL FEES: Parents/guardians qualifying for free or reduced meals are required to sign a fee waiver. After your have completed the enrollment link, your building personnel will need to charge your fees to your account (after checking with Food Service for your free/reduced status). These fees can then be paid either online or at your child's building. View link for student fees.

TRANSPORTATION: There are several pick-up and drop-off town routes around the city of Creston to each of our buildings. Visit this link for the town routes (pick up time is 7:30 a.m. at each stop). If you need transportation regardless whether it is a city route or a rural route, please visit this transportation request link to complete your request.
